Decision Fatigue: Why Repeated Choices Can Push You Toward Shortcuts
Why your choices get worse as the day wears on — you start the morning weighing options carefully, and by evening you're ordering the same takeout again and postponing everything else.
Around four in the afternoon, in clinics across the United States, something odd happens to prescriptions. Patients with colds and coughs — infections that antibiotics can't touch — walk out with antibiotics anyway, noticeably more often than the patients who came in at eight in the morning. Same doctors, same conditions, same guidelines. The most visible difference was where each appointment fell within the clinic session.
That pattern comes from a study of nearly 22,000 patient visits published in JAMA Internal Medicine, and it's one of the clearest real-world patterns associated with decision fatigue: the tendency for decision quality to deteriorate after a long run of choices. The researchers found antibiotic prescribing climbed steadily through each four-hour clinic session, including for infections antibiotics never treat. They describe appointment time as a proxy for accumulated decisions, not a direct measurement of it — ordinary tiredness, time pressure, and patients' own expectations could all be pulling in the same direction. The pattern is consistent with decision fatigue; it doesn't isolate it.
What Decision Fatigue Actually Is
The most careful academic treatment defines decision fatigue as an impaired ability to make decisions and regulate behavior that follows repeated acts of deciding. People experiencing it tend to show three things: they struggle to weigh trade-offs, they drift toward whatever the passive or default option is, and their choices turn more impulsive than usual.
Notice what that list means in practice. Fatigue rarely announces itself as "I can no longer decide." It shows up as deciding differently — grabbing the easy answer, deferring the hard one, or accepting whatever's already in the cart. The antibiotic researchers frame their finding the same way: doctors likely aren't failing to choose so much as drifting toward the path of least resistance, since resisting a patient's expectation takes effort — though the study measured the prescribing pattern itself, not each doctor's reasoning in the moment.
A hypothesized interpretation, not an established mechanism. Several clinical studies found desirable behaviors declining within a work session and partly rebounding near the start of the next one — breaks are one possible explanation, but the studies didn't isolate a break's causal effect from scheduling, workload, or who was seen when.
What the Research Shows — and Where It's Contested
The famous headline finding in this area involves judges: an analysis of over 1,000 parole rulings reported that favorable decisions were far more common right after judges' food breaks than at the end of long deciding stretches. It's a striking result, and it's also seriously disputed. A later simulation study showed that a hypothetical judge with no fatigue at all — one who simply avoids starting a long case right before a break — would produce a downward slope of comparable size, purely from how cases get scheduled and how long they take to hear. That doesn't prove decision fatigue had nothing to do with it, but it means the parole data alone can't carry the claim.
Beyond the antibiotic study, a similar slide has been reported in cancer screening: across 33 primary care practices, both clinicians' ordering of breast and colorectal screening and patients' completion of it decreased as appointment times moved later in the day, with a modest rebound around midday. The study's authors were upfront that this is an association, not a controlled test — clinicians running behind schedule, rushed visits, and patients wanting to leave could all contribute alongside any accumulated decision load. And the theory once used to explain all this — "ego depletion," the idea of willpower as a fuel tank that empties with use — has struggled badly under scrutiny: a preregistered replication across 23 labs and over 2,100 participants found an effect close to zero. What survives is narrower: several healthcare studies have found within-session declines in clinically desirable behavior, while the contribution of decision fatigue specifically is hard to separate from ordinary tiredness, time pressure, and scheduling. That's a real pattern worth planning around. It's not a demonstrated mechanism.
A note on the "35,000 decisions a day" statistic you may have seen elsewhere: it's quoted everywhere, but it doesn't trace back to any actual study. The real research doesn't need inflated numbers — a four-hour clinic session was enough to move prescriptions.
How to Recognize It in Yourself
Decision fatigue is easiest to spot by its fingerprints, not by feeling tired. Watch for the late-day version of you doing these things: accepting defaults you'd normally question ("fine, whatever plan it recommends"), deferring choices that were supposed to happen today, snapping at small decisions that felt easy this morning, or making impulse purchases in the evening that puzzle you the next day. Each one is the trade-off machinery going quiet and the shortcut machinery taking over.
A useful reality check is seeing what your decision-making looks like when it's fresh and deliberate. This test presents everyday scenarios where the best choice follows from the facts — measuring how well you weigh outcomes, ignore sunk costs, and see past framing when nothing is depleted:
What Actually Helps
None of the studies above tested whether these strategies actually improve anyone's real-world decisions — they weren't designed to. What follows are low-cost habits that follow reasonably from the patterns researchers have observed, not countermeasures with their own evidence behind them.
Consider putting the decisions that matter earlier. If the clinical pattern applies to your own day even loosely, trade-off thinking that hasn't been through hours of other decisions first is worth protecting. Contract reviews, purchase comparisons, difficult conversations — morning slots, where possible.
Delete decisions that don't deserve you. The reason some famously busy people wear the same outfit daily isn't fashion philosophy — it's removing choices that consume attention without deserving it. Routines, standing orders, and meal plans all do the same job: they convert recurring decisions into non-decisions.
Consider breaks between decision runs. In the clinical studies, the decline didn't carry over between sessions — each new session started back near baseline, which is at least encouraging: the slide isn't necessarily a one-way ratchet. The antibiotic researchers themselves list breaks as a proposed remedy that needs its own testing, not a proven fix, so treat this as a reasonable thing to try rather than a demonstrated result. Beware the default when you're tired. Fatigued deciders drift toward passivity — and marketers, upsell screens, and auto-renewals are built to catch exactly that drift. If a choice arrives late in your day, ask one question: would morning-you accept this default? Know your gut's failure modes. When deliberate thinking runs low, intuition fills the gap — which is fine, as long as your intuition is well-calibrated. Decision fatigue isn't an excuse for bad choices, and the science behind it is messier than the productivity blogs suggest.
